1. I.D. card.
2. Three one-inch color photos with a glossy single-color background, without a hat, with clear facial features, bust shot, and taken within the past 2 years. Do not use image synthesis (the same as health exam form).
3. One driving license registration form with medical and physical fitness clearance confirmation.

Fees: NT$100.

Notes: The applicant must be at least 18 years old.
