Required Documents:

1. Any of the following identity documents (original copy): National ID card, military ID card, alien residence permit, or another valid driving license.
2. Damaged driving license (not applicable if lost).
3. Two one-inch color photos with a glossy single-color background, no hat, clear facial features, bust shot, taken within the past 2 years. Do not use image synthesis.
4. One automobile (motorcycle) driver review and change registration form.

Fees: Driving license fee: NT$200.

Notes:

1. Please pay any outstanding fines for violations or road safety seminar penalties before applying for license reissuance.
2. If the professional driving license is due for review, please handle the review at the same time.
3. If another person submits the application, they must bring the original copy of their national ID card for verification.
