Required documents:

1. National I.D. card, household registration certificate, or alien residence permit. (If the name and I.D. number are changed at the same time, please submit a household transcript issued within the past three months for verification).
2. The obtained driving license.
3. Proof of I.D. number change issued by the household registration authority (only applicable to those who apply for an I.D. number change).
4. Two one-inch color photos with a glossy single-color background, without a hat, with clear facial features, bust shot, and taken within the past two years. Do not use image synthesis.

Fees:

1. Driving license exempted from renewal (please refer to the description of “periodic renewal of driving license”): I.D. number change due to household registration authority or indigenous people restoring their traditional names: free.
   Any change except for the reasons above: NT$200.
2. Driving license requiring periodic renewal (please refer to the description of “periodic renewal of driving license”): Driving license is not due to expire and I.D. number change due to household registration authority or indigenous people restoring their traditional names: free.
   Any change except for the reasons above: NT$200.
   The driving license is expired: NT$200.
